The KDA has received a few calls regarding the postcard you may have received in the mail, notifying you of two new forms. After speaking with the office of radiation regarding form 07-04, we have been told that this form is intended for physicians who work in more than one facility in order to monitor the total exposure that person is receiving from all sources.
For most dental offices, the dentist must be able to show that the office has been within the exposure limit historically (for example x-ray badges). This historical record will satisfy the KDHE requirements and is good until the changes are made within the office such as a new device, changing of walls, etc. Basically, it does not appear that there is anything new for the dental offices.
